Changing Phases Behavioral Health is seeking to immediately hire Substance Abuse Counselors that are CAC certified. POSITION REQUIREMENT: Candidate must have an undergraduate degree in a related field and be CAC certified or hold an LPC, LCSW license. If Counselor is not a CAC, LPC or LCSW, that Counselor will be required to diligently work toward certification while in the employment of Changing Phases Behavioral Support. Candidate must be willing to work Monday-Friday from 5:00 am-1:00 pm. Intake Evaluate patient and determine appropriateness of MOUD admission per State and Federal criteria. Provide information about MOUD and alternative treatments to ensure that the patient is able to give informed consent. Review program rules and regulations; provide basic program orientation Complete psychosocial and treatment plan appropriate to patient's condition Treatment Work with patient to develop individualized treatment plan and goals Obtain urine drug screens for every patient at least once a month on a random schedule. Annually obtain proof of residence and proof of income from every patient on caseload. Complete in the appropriate time frame treatment plan reassessment for every patient. Complete and submit all patients' requests to the MD. Make certain that patient records are always in the proper order and are stored in a confidential manner Meet with assigned patients as determined Record all counseling sessions appropriately for documentation purposes. Additional Responsibilities Arrange guest dosing for patients as needed Complete minimum required trainings as directed. Conduct the Patient Orientation class as required.
