A company is looking for a Family Nurse Practitioner (Virtual - Illinois Licensed).
Key Responsibilities

Conduct virtual wellness visits and health assessments for patients
Collaborate with in-home clinical team members to provide comprehensive care
Manage care coordination, including preventive care interventions and referrals

Required Qualifications

Board Certified as a Family Nurse Practitioner in Illinois
Active Family Nurse Practitioner License in Illinois
Exhibits professionalism, integrity, and compassion in healthcare delivery
Technologically savvy with experience using digital tools for patient care
Ability to work autonomously and adapt to a dynamic work environment
