Job Description:
A company is looking for a Remote LCSW.
Key Responsibilities
Conduct comprehensive psychosocial assessments to identify patient
needs and challenges
Provide counseling and develop personalized care plans in
collaboration with patients and healthcare teams
Document patient interactions and coordinate with the clinical team
to ensure exceptional patient experiences
Required Qualifications
LCSW license with the ability to practice in NY, FL, and MI
2+ years of experience as an LCSW
Bachelor's and Master's degrees required
Experience with telehealth platforms and electronic health record
(EHR) systems
Passion for facilitating behavior change and managing chronic
disease
