  The Registered Nurse (RN) in Home Health provides nursing care to patients in their homes, as prescribed by the physician, within applicable laws, regulations, and agency policies.

  • Coordinates total plan of care with health care professionals, achieving and maintaining continuity of patient care.
    
  • Develops and implements plans of care, documents accurately and timely.
    
  • Ensures patients meet home health eligibility and medical necessity guidelines.
    
    Requirements:
    
    • 
      
    • Current RN licensure in state of practice
      
    • Current CPR certification
      
    • Current Driver's License, vehicle insurance, and dependable vehicle or public transportation
