Full Description Nurse Practitioner (Bilingual Spanish) - Atlanta, GA Shift: Full-time (Days)Job Summary:
Field-based, making Primary Care House Calls, Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes, Collaborate with a multidisciplinary team to deliver integrated care. Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients, Fluent Spanish is ideal.Job Responsibilities:

• Field-based, making Primary Care House Calls.
• Conduct comprehensive assessments, develop care plans, and provide continuous care for older adults in their homes.
• Collaborate with a multidisciplinary team to deliver integrated care.
• Utilize your expertise in geriatrics and primary care to make a meaningful difference in the lives of your patients.
• Fluent Spanish is ideal.Qualification/Experience:
  • Valid Georgia ARNP license.
  • Reliable transportation and current state driver's license and proof of auto insurance.
  • Current DEA license.Medical Specialty:

      • It involves providing primary care for older adults (65+) in their homes (house calls). This is not home health or home care, but rather primary care delivered at home.
