Job Description:

A company is looking for an LTSS Service Care Manager (RN) to develop and coordinate care management activities for high-risk populations.

Key Responsibilities

Evaluate the medical, behavioral, and social needs of complex members and recommend care plans
Act as a liaison between members, families, and healthcare providers, performing frequent visits to assess needs
Educate members and families on healthcare services, benefits, and community resources

Required Qualifications, Training, and Education

Graduate from an Accredited School of Nursing or a Bachelor's degree
2 - 4 years of related experience
Current RN or NP licensure in the state
Experience in long-term care management preferred
Must reside in Oahu
